Health Insurance Cost Sharing: Requiring specified individual health insurers and their pharmacy benefits managers to apply payments by or on behalf of insureds toward the total contributions of the insureds’ cost-sharing requirements; requiring specified individual health insurers to disclose on their websites and in their policies their applications of payments by or on behalf of policyholders toward the policyholders’ total contributions to cost-sharing requirements; requiring pharmacy benefits managers to apply payments by or on behalf of insureds and subscribers toward the insureds’ and subscribers’ total contributions to cost-sharing requirements, etc.
